Beacon Mental Health’s Adult Healthcare Home (HCH) provides comprehensive support for adults managing chronic physical and behavioral health conditions. HCH is not a home health care service or a place of residence; it is a coordinated care model focused on improving long‑term health outcomes through partnership, prevention, and personalized care.
A Dedicated Team Working for You
Our Adult Healthcare Home team includes:
- Three Nurse Care Managers
- A Nurse Care Coordinator
- A Specialized Healthcare Consultant
- A Medication Services & Healthcare Home Supervisor
- A Director of Healthcare Home & Nursing
This multidisciplinary team ensures that each adult receives the care, resources, and guidance they need to maintain health and stability.
Core Services
Nurse Care Management
Each participant works one‑on‑one with a Nurse Care Manager (NCM) who provides:
- Wellness coaching and healthy‑lifestyle guidance
- Education and support for managing chronic illnesses
- Connections to primary care providers, including Swope Health
- Tobacco cessation services (from certified cessation specialists)
- Screenings for cholesterol, blood glucose, and Hemoglobin A1c
- Blood‑pressure and weight‑management support
- Help with post‑hospitalization care, including medication reconciliation
Nurse Care Managers partner with individuals to identify goals, reduce health risks, and improve overall wellness.
Care Coordination
Our Nurse Care Coordinator assists with:
- Scheduling medical, behavioral health, and specialty care
- Ensuring follow‑up and continuity
- Reducing barriers to accessing care
Specialized Healthcare Consultation
The Specialized Healthcare Consultant reviews every enrolled client and works with the team to ensure cohesive, whole‑person care planning.
